To the non-profit entities of a union nature that bring together production entrepreneurs domiciled in Bogotá D.C. to present a list of candidates, for the election of their representative before the Board of Directors of the Integrated Subnet of Health Services Central East, North and South E.S.E. (Chamber of Commerce certificate) no longer than thirty (30) days.

• Certification issued by the Chamber of Commerce, which states that non-profit entities of a union nature are not undergoing dissolution and liquidation.

• Group of production entrepreneurs, who develop activities in the area of ​​influence of the corresponding Integrated Subnetwork of Health Services E.S.E. for which the representative of the production unions will be elected. This will be validated with the list that the Chamber of Commerce will provide to approve the production entrepreneurs in the area of ​​influence of the subnetwork.

• Comply with the guidelines of Chapter V, Production Unions of Directive 001 of 2026, issued by the District Health Secretariat of Bogotá D.C. 2. Nominated candidates:

The candidates from the shortlists nominated by the production unions must meet the following requirements: National Decree 780 of 2016, article 2.5.3.8.4.2.4:

• Be linked to and fulfill specific health functions in a Committee of Health Users Health Services; prove work experience of no less than one (1) year in a User Committee.

• Not being subject to any of the disabilities and incompatibilities contemplated in the Law.

Comply with the guidelines of Chapter V, Production Unions of Directive 001 of 2026, issued by the District Health Secretariat of Bogotá D.C., which must be taken into account for the application.

3. Date of receipt of the documents:

The non-profit entity of a union nature, through its legal representative, by letter addressed to the District Secretary of Health, will present the complete resumes of its shortlist with their respective supports, attaching a letter of acceptance as a member of the respective Board, if elected, and other documents that prove compliance with the stated requirements. The appointment will be made within the next ten (10) business days, counted from the closing of the deadline to present shortlists, after evaluation of the respective resumes presented by the trade organizations of their shortlists of candidates. • Photocopy of the citizenship card.

• Certificate of Disciplinary Record (Attorney General's Office).

• Certificate of Disciplinary Record (Personaje of Bogotá D.C.).

• Certificate of Tax Records (Comptroller General of the Republic).

• Certificate of Judicial Records (National Police of Colombia), with an issue date of no more than one (1) month.

• Certificate of National Registry of Corrective Measures.

• Certificate of Registration of Delinquent Food Debtors – REDAM

• Acceptance letter addressed to the District Secretary of Health.

• Certification of length of experience, functions performed and affiliation issued by the respective organization it represents.

• Communication signed by the delegate and addressed to the District Secretary of Health, in which he accepts the representation and declares that he is not subject to any of the disabilities and incompatibilities of the Law to be held and serve as a representative before the Board of Directors of the Integrated Subnetwork of Health Services -E.S.E.

• Copy of the minutes of the general election assembly or the meeting to form shortlists in the case of production unions and assistance supports in all cases.

• Copy of the constitution document of the organization it represents or certificate of existence and legal representation, as applicable, stating that it is not in a state of dissolution and liquidation.
